Medical Clearance 

As a student-athlete at Bates College within the NESCAC and NCAA, you are required to complete the following steps to become eligible to participate in any athletic activity at Bates College. Athletic activity includes all practices, games, performance testing, and strength & conditioning sessions. Bates Athletics and Bates Sports Medicine utilize the Teamworks app for all paperwork, appointment scheduling, and messaging.

Medical Paperwork Requirements


New Student-Athletes (Incoming or Transfer):

AMS Portal Information:
    -Profile Form
    -Health Insurance Questionnaire
    -Medical History Questionnaire
    -PHQ-8
    -ADD/ADHD Verification Waiver
        -If you take ADD/ADHD medication, your doctor must complete a specific NCAA ADD/ADHD form.
    -Central Maine Medical Center Risk/Consent to Treat Form
    -NCAA Concussion Fact Sheet
    -NCAA Videos
    -Sickle Cell Trait Status Form
        The NCAA requires confirmation. Get your results from your doctor or birth hospital. If you can not locate your results with your doctor's office and you were born in the United         States, try contacting the newborn screening program of the state you were born in. If you are unable to obtain results, get tested before you arrive. You must upload the test         results.

Pre-Existing Injuries:
If you had an injury in the 12 months that stopped you from playing, you must upload all medical records (doctor’s notes, surgical notes, and a clearance letter to return to sport)

Returning Student-Athletes:
