Please use the checklist below to ensure all necessary paperwork has been completed and successfully submitted to the Bates Sports Medicine staff before reporting to campus for participation in your respective sport. Incomplete or inaccurate paperwork will result in a failed pre-participation examination preventing your medical clearance to compete in your sport.
